COVID-19 Vaccination Questionnaire

Quickly and easily track whether employees have been vaccinated for COVID-19 through HireRight’s electronic medical questionnaire, which follows the guidelines established by the CDC. The information collected includes the vaccine manufacturer, associated dosage dates, and uploaded proof of vaccination. Reports are available in real-time on HireRight’s Screening Manager platform. Premier level service available to verify that the vaccination information entered matches the card documentation provided.

 

Instant Antigen Tests*

HireRight’s instant COVID-19 antigen tests help employers comply with OSHA’s Emergency Temporary Standard, which requires weekly testing for unvaccinated workers. The lower nasal swab tests can be easily administered on-site, with results available in 15-30 minutes. Employers can order test kits in bulk quantities or small batches to accommodate their scheduled testing requirements. Access to historical test results can be accessed at any time in HireRight’s Screening Manager and serves as documentation to support compliance requirements.

 

Viral Marker Tests (RT-PCR**)

Self-administered viral marker tests (saliva tests) can be performed in the safety and comfort of an employee’s home, no need to schedule a clinic or telemedicine visit.

Individual test kits can be sent in bulk to the employer for distribution or sent directly to the employee. Optional counseling from physicians is available for employees who test positive. Viral marker tests accommodate:

  • Ad Hoc Testing – testing as needed for employees displaying symptoms or who have been exposed to someone that has been diagnosed with the virus.
  • Assurance Testing – ongoing testing conducted for the entire workforce at periodic intervals. Assurance testing helps identify cases of COVID-19 early, especially among employees who are either pre-symptomatic or asymptomatic without outwards signs of infection.

 

COVID-19 Self-Assessment Tool (provided by Preworkscreen***)

A simple and efficient way to check for COVID-19 symptoms before employees begin their workday. Employees can download the Preworkscreen app on their phone and complete the self-assessment on-the-go. Questions are based on the most current CDC and state-specific guidelines. Assessment results are compiled in real-time and immediately available to employers through their Preworkscreen dashboards, where they can quickly track and monitor the overall health statuses of their workforce.
