COVID-19 Vaccination Questionnaire
HireRight can help you quickly and easily track whether your employees have been vaccinated for COVID-19 through our electronic medical questionnaire, which follows the guidelines established by the CDC. The information collected includes the type of vaccination given (Pfizer, Moderna, Johnson & Johnson, etc.) and the associated dosage dates for the first, second and third (booster) shots if applicable. Employees are then required to upload their COVID documentation.

COVID-19 Viral Marker Tests
HireRight offers simple, self-administered viral marker* tests that can be performed in the safety and security of an employee's home, no need to schedule a clinic or telemedicine visit. The test detects the presence of coronavirus in a saliva sample that is easy to collect and is not "technique dependent".

Questions are based on current Center for Disease Control and Prevention (CDC) and state specific guidelines where employees are located.
Assessment results are compiled in real-time and immediately available to employers through their Preworkscreen dashboards where they can quickly track and monitor the overall health-statuses of their workforce.
